Solera Senior Living Unveils Innovative Multi-Partner Care Model at Lumina Las Vegas, Cutting Falls and Hospitalizations

Solera Senior Living, a Denver-based operator, developer and investor of next generation senior living communities across the U.S, today announced the launch of the first pilot of the Alliance for Connected Senior Care, a new collaboration among five industry leaders – ALIS, EmpowerMe Wellness, Inspiren, Senior Doc, and Solera – working together to test a groundbreaking model for resident-centered care.

The pilot, launched at Solera’s Lumina Las Vegas Memory Care community, is designed to improve outcomes through coordinated communication and shared data. As part of the model – which would not be possible without the vision of the Las Vegas health care community’s renowned Steinberg family – care teams align workflows and hold weekly cross-company clinical calls to provide holistic, resident-centered support and avoid unnecessary ER visits.

Early results are strong: Lumina Las Vegas has seen a 57% reduction in falls, a 54% drop in ER visits, and a 44% decrease in hospitalizations compared to January–August baseline metrics.

“We’re proving that senior living communities can evolve care models to drastically improve resident outcomes” said Adam Kaplan, Founder and CEO of Solera Senior Living. “By working with our strategic partners, we’ve shown that resident-centered communication and AI-supported workflows can make a real difference.”

Each partner contributes a distinct capability to the pilot, combining to create a connected model of care. Key elements of the pilot model include:

  • Senior living’s first AI-powered ecosystem, Inspiren, which empowers care teams with real-time behavioral alerts, fall detection, and ongoing care utilization
  • Industry leading on-site Senior Living care delivery model from Senior Doc driving value based care outcomes and quality metrics through tech-enabled care services and integrations.
  • Coordinated therapy planning through EmpowerMe Wellness, allowing care teams to tailor recovery and prevention plans to each resident’s needs
  • Secure, HIPAA-compliant data sharing via ALIS’ eHR software, under resident consent protocols
  • Weekly cross-company clinical calls for proactive planning, ensuring shared decision-making across all care disciplines

The pilot has already demonstrated faster incident response, smoother coordination among staff and clinicians, and stronger recovery and prevention protocols. By integrating insights from Senior Doc and EmpowerMe, care teams can proactively adjust therapy and clinical plans, while AI-supported safety awareness from Inspiren enables real-time interventions. Secure data sharing through ALIS ensures all partners are aligned, enabling more personalized, timely, and effective support for residents.

Alliance for Connected Senior Care represents a blueprint for industry-wide changes, moving beyond single-technology implementations to a truly collaborative, collective, multi-partnered approach. It will continue to refine its program and expansion strategy while tracking key performance metrics through Q1 2026 to further validate the model’s impact and scalability. This allows the Alliance to establish new standards of care and pull the industry forward by working together.
